Pediatric Home and Community Care |
This working group focuses on a foundational strategy for the Kids Come First Health Team. Family partners, home care agencies, physicians caring for children with complex medical needs, and community organizations work together to co-design improvements to home care service delivery models. Long-term goals for this initiative included creating a more holistic approach to care, enhancing access to services, and providing a better system of seamless and coordinated care. |
Mental Health, Addictions, and Substance Use Health Table |

Using a regional data-driven approach, this table can identify priority areas, where working together partners can transform the health care system for access, navigation as well as infant early childhood mental health, integration in the delivery of services for mental health and substance use health, and building capacity of the workforce. |
Population & Public Health |
The population and public health working group is a coalition of individuals and organizations who understand that a population health approach is the glue that should hold together individual healthcare transactions. Their goal is to improve the health of the entire population and to reduce health inequities among population groups. |
