Kids Come First Multimedia Consent Form

PLEASE READ CAREFULLY

Please complete the Kids Come First Multimedia Consent Form to permit the use of image or likeness (photos, video, audio, etc.), release of information of child, youth or family, staff, volunteers, or other for use in Kids Come First websites, social media channels, publications, marketing materials and internal channels, news media outlets (print, radio, television, online and social media)  or other as indicated on the form.

Completing the Kids Come First Media Consent Form hereby grants Kids Come First Health Team and its affiliates shall be the sole owner of such materials and information including all copyright.

If you wish for Kids Come First Health Team to stop using or limit the use your child’s information please contact communications@kidscomefirst.ca. Please note that once your child’s information, video(s) or photo(s) is shared in print publication(s), on website(s) and social media it becomes public knowledge and will no longer be protected. It may still be visible on the internet even after Kids Come First Health Team has removed it from its own channels. Other people or organizations who accessed and viewed it might also use or share it.
